Before Participating in Practice All Necessary Forms Must Be Compleated and Turned In. See "Forms Section" Below.
SMS ASB Fees:
Students/parents need to pay Tammy Voelz, our SMS bookkeeper, the $30 ASB fee. ASB fees are required for participation in SMS activities. Upon payment students will be provided an ASB Card “sticker” which grants them access to all home events.
K -12 Accident Insurance Coverage
FORMS :
Registration and mandatory forms must be completed prior to first practice.

Sandpoint Middle School Athletics Sign-Off Form (Every year.)
All 7th-graders and any NEW to SMS 8th-grade students will also need to have a physical examination performed by a medical professional and turn in a completed Extra-Curricular Health Form before the first day of practice.
Permission To Travel With Another Parent Form (Due 48 hours in advance of road trip)
