📝 Before Participating in Practice

All required forms must be completed and submitted prior to attending any practice. Please see the Forms Section below for access.

💳 SMS ASB Fee

A $30 ASB fee is required for all students participating in SMS activities.

• Payment should be made to Tammy Voelz, SMS Bookkeeper.
  
  

• Students will receive an ASB card sticker upon payment, which grants access to all SMS home events.
  
  

🛡️ K–12 Accident Insurance Coverage

For optional coverage, please review the Student Accident Insurance Information provided by the district.

📄 REQUIRED FORMS

All forms must be submitted before the first day of practice:

• SMS Athletics Sign-Off Form (required every year)
  
  

• Extra-Curricular Health Form
  – Required for all 7th-grade students
  – Required for new-to-SMS 8th-grade students
  – Must include a physical examination performed by a licensed medical professional
  
  

• Permission to Travel with Another Parent Form (must be filled out by all parties and submitted 48 hours before travel)
