📝 BEFORE PARTICIPATING IN PRACTICE
All required forms must be completed and submitted before a student may participate in practice.
Please review the Required Forms section below and make sure everything is completed before the first day of practice.
📄 REQUIRED FORMS
Required every year.
Required for:
All 7th-grade students
New-to-SMS 8th-grade students
Must include a physical examination performed by a licensed medical professional
Must be completed by all required parties and submitted at least 48 hours before travel.
💳 SMS ASB FEE
A $30 ASB fee is required for all students participating in SMS activities.
Payment should be made to Tammy Voelz, SMS Bookkeeper.
Students will receive an ASB card sticker upon payment, which provides admission to all SMS home events.
🛡️ K–12 ACCIDENT INSURANCE COVERAGE
Optional student accident insurance coverage is available.
For coverage information, please review the Student Accident Insurance Information provided by the district.
