A $30 ASB fee is required for all students participating in SMS activities.
Payment should be made to Tammy Voelz, SMS Bookkeeper.
Students will receive an ASB card sticker upon payment, which grants access to all SMS home events.
All forms must be submitted before the first day of practice:
Permission to Travel with Another Parent Form (must be filled out by all parties and submitted 48 hours before travel)
